PFAS in Baby Formula and Infant Exposure

Multiple Infant Exposure Pathways

Infants face PFAS exposure through multiple routes simultaneously: breast milk from PFAS-exposed mothers, formula reconstituted with PFAS-contaminated tap water, and direct ingestion of other water-based foods. Infant body burden per unit of body weight can exceed that of adults because infants consume proportionally more water-based liquids, PFAS passes through the developing gut more readily, and the developing organ systems are more sensitive to PFAS toxicity than mature adult systems.

Formula Preparation Water Quality

Reconstituted powdered formula prepared with PFAS-contaminated tap water can deliver a significant PFAS dose to infants at each feeding. The EPA’s maximum contaminant levels are set as health limits for adults, but no specific infant health advisory has been established, and the appropriate level for infant exposure may be lower given developmental vulnerabilities. Families in areas with detected PFAS in municipal water are advised to use certified PFAS-free water for formula preparation.

Testing of Commercial Formula Products

Independent testing of commercial formula products — including ready-to-feed liquid formulas — has found PFAS in some products. Ready-to-feed formulas are more concentrated in PFAS content relative to their liquid volume than powdered formulas reconstituted with treated water, because the liquid base cannot be substituted. The FDA has tested formula products for PFAS as part of its total diet study, but comprehensive surveillance data on PFAS in infant formula is limited.

Practical Guidance for Parents

Parents in areas with known or detected PFAS in tap water should use certified bottled water or filtered water from a system verified for PFAS reduction when preparing powdered infant formula. Reverse osmosis filtration with NSF/ANSI 58 certification and activated carbon block systems certified under NSF/ANSI 53 for PFAS reduction are appropriate choices. Testing treated water to confirm PFAS reduction before relying on it for infant formula preparation is the most confident approach available to concerned parents.
