PFAS and Fibromyalgia: Neuroinflammation and Pain Sensitization

Fibromyalgia: Chronic Pain Without Clear Pathology

Fibromyalgia affects approximately 2–8% of the global population, predominantly women, causing widespread musculoskeletal pain, fatigue, and cognitive difficulties. The pathophysiology involves central sensitization and neuroinflammation.

PFAS and Neuroinflammatory Pathways

PFAS compounds activate microglia, the brain’s immune cells, triggering neuroinflammatory cascades that sensitize pain processing centers. Elevated inflammatory cytokines including TNF-alpha and IL-6 are found in both fibromyalgia patients and PFAS-exposed individuals.

Hypothalamic-Pituitary-Adrenal Axis Disruption

PFAS disrupt cortisol regulation via HPA axis interference, impairing the body’s stress response and pain modulation mechanisms. Abnormal cortisol awakening responses are documented in both fibromyalgia and PFAS-exposed populations.

Integrative Management

Pain specialists should consider PFAS exposure history in fibromyalgia evaluation. Anti-inflammatory interventions, including clean water access and reduced PFAS dietary exposure, may complement standard multimodal fibromyalgia treatment.
